Patient Information
South Hills Physical Therapy Clinic is “cash pay”, meaning payment is due at time of treatment. I accept cash or check as a form of payment. Sorry, I do NOT accept credit or debit cards at this time. If you have current insurance that will cover your treatment I will prepare a receipt with all the appropriate information needed for you to submit to your insurance company for direct reimbursement. Please call for information regarding current treatment rates and referral requirements.
Animal Physical Therapy Patients:
All animals in Oregon require a veterinarian referral before being treated in the clinic by an animal physical therapist, and which must be renewed every six months. There are referral forms available on the Forms page that can be downloaded and faxed back to your veterinarian.
Human Physical Therapy Patients:
All PT patients require a physician referral 60 days after first treatment date. Referrals must be kept current for ongoing treatment. Some insurance companies require a referral from the first visit in order to be reimbursed for treatment. Please check with your insurance company for your referral requirements and pre-approval status. I am not a Medicare provider and cannot accept Medicare patients for treatment. I also do not accept workman’s compensation patients at this time. Motor vehicle accidents (MVA) will be accepted on a per case basis.
Nutritional Therapy Clients:
A Nutritional Therapy Practioner is trained to evaluate your nutritional needs and make recommendations of dietary change and nutritional supplements. A Nutritional Therapy Practitioner is not trained to provide medical diagnoses, and no comment or recommendation should be construed as being a medical diagnosis. Some Health Savings plans will reimburse clients for our services.
Appointments
Your appointment time is scheduled based on the concern you express at the time of your call. For new human physical therapy patients, I might schedule two to four appointments in a row, so I am sure you will have continuing care in a timely manner.
First Appointment
If you haven’t downloaded and completed the new patient forms, please arrive 10-15 minutes prior to your new appointment to do so. In certain situations, I can mail the paperwork to you in order to have the forms completed and your appointment time is only addressing your concerns.
Please bring with you:
1. Paperwork and/or X-rays
2. Insurance card
3. Photo ID
4. Current doctor or veterinary prescription
5. Claim number or pre-authorization form is needed for insurance
6. MVA: Insurance information, claim number, claim adjusters name and address
7. Check or cash, sorry no credit or debit cards accepted.
Cancellations
I need at least 24-hours notification of your cancellation so that another patient in need can be seen. Failure to provide adequate notice of cancellation may result in a charge for the missed appointment, unless another person is found to take your place, or termination of medical services by the clinic.
Animal Appointments
If you schedule an animal appointment, please leave your animal in the car, or outside, and let me know you have arrived. Animals have their own entry door to the clinic.
