Appointments

Our office works on an appointment basis depending on which therapist you see at the time. We do our best to accommodate your special needs at the time of scheduling. We understand and recognize the importance of your time. To help ensure we see you in a timely fashion and address your concerns, we would like to share the following information.

Your appointment time is scheduled based on the concern you express at the time of your call. For new human physical therapy patients we might schedule four appointments in a row so we are sure you will have continuing care in a timely manner.

First Appointment
Please keep in mind that new appointments require time for filling out paperwork. Please arrive 10-15 minutes prior to your new appointment to accomplish this task. This will help ensure that your appointment time is spent addressing your concerns.

If you wish to fill out the paperwork at a time that is more convenient for you, you may request that we mail the paperwork to you before your appointment.

Please bring with you:

  1. Paperwork and/or X-rays
  2. Insurance card
  3. Photo ID
  4. Current doctor or veterinary prescription
  5. Claim number or pre-authorization form if needed for insurance
  6. MVA: Insurance information, claim number, claim adjusters name and address
  7. Check or cash, sorry no credit cards, as we are "cash for service" clinic

Cancellations
Since we have reserved time for your appointment, we need at least 24 hours notice of cancellation so that another patient in need can be seen.

Failure to provide adequate notice of cancellation may result in a charge for the missed appointment(s) or termination of medical services by our office.

Animal Appointments
If you scheduled an animal appointment, please leave your animal in the car or in our "doggie waiting room" outside, and let our staff know you have arrived. Animals are not allowed in the human waiting room area with the exception of service dogs.
